Presbyterian is seeking a Home Health, Hospice & Palliative Liaison to navigate clinical referrals under the direction of the Patient Care Managers. The role coordinates field clinicians’ patient visits to meet regulatory specifications from referring providers and schedules staffing for all departments to maintain productivity. Responsibilities include managing the intake workflow, scheduling complex referrals, and supporting centralized agency scheduling tasks for CDS and Regional Agency operations, including Hospice Face to Face tracking, Benefit Period tracking, supervisory visits reports, and hospice notices of elections.
Responsibilities
• Utilizes EMR scheduling applications to manage patient appointments and validate demographics.
• Coordinates scheduling for RDS Home Health and Hospice, including provider block time across sites.
• Ensures compliance with CMS guidelines for clinician scheduling and documentation.
• Maintains positive relationships with statewide agency personnel, internal and external stakeholders.
• Handles after-hours/weekend pass-downs and manages clinician schedules using time and attendance systems.
• Produces required reports such as Supervisory Visits and Geographic Team Reports.
• Answers agency phone queue, resolves inquiries, and supports message routing.
• Supports onboarding by managing EMR schedule templates and mentoring new CSC staff.
• Secures signed provider orders to ensure timely billing and regulatory compliance.
Qualifications
• High School Diploma or GED required; Associate degree preferred.
• Two or more years of clinical experience as a Clinical Scheduler required.
• Short-term training in Medical Terminology.
• Customer service and scheduling experience.
• Advanced computer skills with the Microsoft Office platform.
