Job Summary:

We're looking for a full-time, bilingual (English and Spanish), and remote LVN/LPN to provide high-quality patient care, coaching, and appointment scheduling via phone. As a Care Coordinator, you will:

  • Remotely deliver patient health coaching and care to improve patient outcomes
  • Coach and educate patients on managing chronic conditions, preventive care, and physician directives
  • Update and manage patient care plans based on assessments and physician directives
  • Maintain scheduling flexibility between 8:00am - 7:00pm (PST) to accommodate patients in different time zones

Requirements:

  • Valid LVN/LPN license in the state of New York (mandatory)
  • 2+ years of experience as a Licensed Vocational Nurse (LVN/LPN)
  • Bilingual proficiency in English and Spanish
  • Experience working with Electronic Health Records (EHRs) and Electronic Medical Records (EMRs)
  • Strong knowledge and background in chronic conditions
  • Excellent patient rapport-building skills
  • Compassionate and caring demeanor
  • Strong documentation and verbal communication skills
  • Intermediate to advanced computer skills with the ability to navigate multiple systems
  • Reliable internet connection
