Maximus is seeking a compassionate and experienced Registered Nurse Assessor to join our team in the Bronx, NY. As a hybrid remote role, you will have the flexibility to work from home and conduct in-home assessments throughout the assigned region. Key Responsibilities:

• Conduct comprehensive assessments to determine eligibility for community-based long-term services
• Render medical necessity determinations and resolve technical issues
• Perform initial assessments, evaluate health status, and document outcomes
• Maintain current knowledge of regulatory and project changes
• Collaborate with interdisciplinary care team members to facilitate receipt of services

Requirements:

• Active RN license in the state of New York
• Minimum 2 years of clinical experience with RN license
• Minimum 2 years of home setting experience, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional
• Ability to conduct in-home health assessments and travel within assigned region
• Proficiency in navigating web-based applications and Microsoft Outlook & Word

Home Office Requirements:

• Reliable high-speed internet service
• Minimum 20 Mbps download speeds/50 Mbps for shared internet connectivity
• Minimum 5 Mbps upload speeds
• Candidates must have their own cell phone
