Registered Nurse (PRN) - Transitional Care Management - $30/hr

We are seeking a highly skilled and compassionate Registered Nurse to join our team on a PRN basis. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care. As a Transitional Care Management Registered Nurse, you will be responsible for coordinating and managing patient transitions, developing individualized care plans, and educating patients and caregivers on self-care practices and medication management.
Key Responsibilities:
- Coordinate and manage patient transitions from hospital to home or other care settings, ensuring continuity of care and reducing readmission rates.
- Collaborate with healthcare providers, patients, and families to develop personalized care plans that meet unique needs and goals.
- Educate patients and caregivers on self-care practices, medication management, and follow-up appointments to promote optimal health outcomes.
- Monitor and evaluate patient progress, making adjustments to care plans as necessary to ensure effective care coordination.
- Ensure timely follow-up with patients and primary care providers to address any issues or concerns, providing ongoing support and guidance throughout the transition process.
- Document all patient interactions, assessments, and care plans in accordance with hospital policies and procedures, maintaining accurate and up-to-date records.
- Participate in interdisciplinary team meetings to discuss patient progress and discharge planning, contributing to a collaborative and patient-centered approach to care.
- Serve as a resource for patients and families, providing emotional support, guidance, and education to empower informed decision-making and self-advocacy.
- Maintain current knowledge of best practices in transitional care management, staying up-to-date on the latest research, guidelines, and industry developments.
Qualifications:
- Current RN license in FL or compact license that allows work in FL, with a minimum of 2 years of experience in a clinical or hospital setting.
- Preferred experience in transitional care management or case management, with strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong organizational skills and attention to detail, with the ability to manage multiple priorities in a fast-paced environment.
- Knowledge of electronic health records (EHR) systems and proficiency in documentation, with the ability to maintain accurate and up-to-date records.
Work Schedule:
This is a PRN position, with hours varying based on the needs of the department on an as-needed basis. Flexibility is required to accommodate patient needs, with the opportunity to work remotely from home and collaborate with our team in the office.
