Available SchedulesWe have openings for the following shifts on Saturday - Sunday:7:00am - 3:30pm3:30pm - 7:00pmReliable transportation is required. Please indicate your preferred schedule when applying.Key Responsibilities

• Personal Care: Assist patients with bathing, grooming, dressing, skincare, and mobility support.
• Meal Preparation: Plan and prepare nutritious meals tailored to dietary needs.
• Household Assistance: Maintain a clean, safe, and healthy environment for patients.
• Documentation: Accurately record patient care activities using Celltrak.
• Team Collaboration: Report changes in patient conditions to RN Case Managers.

Qualifications

• Certification: Valid NJ Certified Home Health Aide (CHHA) in good standing.
• Communication Skills: Proficient in reading, writing, and communicating fluently in English.
• Physical Requirements: Ability to lift, push, and pull up to 50 lbs.
• Travel: Willingness to travel within Gloucester County.
