Remote Senior Social Worker - Case Management
Our client is seeking an experienced and compassionate Remote Senior Social Worker to join their dedicated team, focusing on comprehensive case management services for vulnerable populations. This is a fully remote position, allowing the successful candidate to work from anywhere within the United States. You will play a crucial role in assessing client needs, developing service plans, coordinating resources, and advocating for clients to ensure they receive appropriate support and achieve positive outcomes. Key Responsibilities:
Conduct comprehensive assessments of client needs, including social, emotional, environmental, and medical factors. Develop individualized service plans in collaboration with clients and their families. Coordinate and arrange for the provision of necessary services, such as housing, healthcare, employment assistance, and counseling. Provide ongoing case management, monitoring client progress and adjusting service plans as needed. Advocate for clients' rights and ensure access to community resources and benefits. Maintain accurate and confidential client records in compliance with agency policies and legal requirements. Collaborate with other professionals, agencies, and service providers to ensure holistic client care. Provide crisis intervention and support to clients experiencing emergencies or significant distress. Educate clients and families about available resources, programs, and treatment options. Stay current with relevant laws, regulations, and best practices in social work and case management. Participate in team meetings, case consultations, and professional development activities. Required Qualifications:
Master's degree in Social Work (MSW) from an accredited institution. Current and unrestricted Licensed Clinical Social Worker (LCSW) or equivalent state licensure is required. Minimum of 5 years of post-MSW experience in clinical social work and case management. Proficiency in conducting comprehensive client assessments and developing effective service plans. Strong knowledge of community resources, social services, and relevant legal frameworks. Excellent interpersonal, communication, and counseling skills. Ability to work independently, manage a caseload effectively, and maintain strong organizational skills in a remote setting. Proficiency with electronic health record (EHR) systems and other relevant technology for remote work. Experience in crisis intervention and de-escalation techniques. Demonstrated commitment to diversity, equity, and inclusion. Ability to set boundaries and maintain professional ethics. Familiarity with specific populations (e.g., child welfare, mental health, elderly care) is a plus.
