Remote SNF Case Manager
Location: Fully Remote
Schedule: Monday – Friday | 7:00 AM – 4:00 PM CST
Serenity Estates is seeking an experienced Skilled Nursing Facility (SNF) Case Manager to join our growing team in a fully remote role.
This position is responsible for coordinating and managing the insurance authorization process for SNF admissions and continued stay reviews, working closely with facility teams and payers to ensure timely approvals and optimal resident care coordination.
Key Responsibilities
• Submit insurance authorizations for SNF admissions
• Manage and track continued stay reviews
• Communicate with insurance providers and facility teams regarding authorization status
• Ensure documentation meets payer requirements
• Collaborate with admissions, therapy, and nursing teams to support appropriate length of stay
• Maintain accurate records of authorizations and payer communication
Qualifications
• Active RN, LPN, COTA, PTA license preferred
• Skilled Nursing Facility case management experience REQUIRED
• Experience submitting SNF authorization requests and continued stay reviews
• Knowledge of Medicare Advantage and managed care payers
• Strong organizational and communication skills
• Ability to work independently in a fully remote environment
