Nurse Navigator Oncology - REMOTE

In this role, you will play a pivotal role in coordinating and implementing strategies aimed at ensuring quality cancer care for patients across various settings. You will be responsible for assessing patient needs, providing education and resources, and facilitating informed decision-making throughout the entire oncology continuum. Your collaboration with multidisciplinary teams and communication with treating physicians will be crucial in delivering exceptional patient care. The role also involves evaluating community needs and engaging in program development to enhance oncology services offered by the organization. Responsibilities • Coordinate strategies for quality and arenaflex-effective oncology patient care. • Assess patient needs and orient them to hospital and outpatient settings. • Deliver comprehensive cancer care from diagnosis to survivorship. • Educate patients and families to support informed decision-making. • Collaborate with multidisciplinary teams and managing new patient referrals. • Develop and report metrics to enhance oncology programs. • Provide emotional support and advocate for patients in their journeys. Requirements • Registered Nurse (RN) licensure required. • Bachelor of Science in Nursing (BSN) preferred. • Minimum of 5 years of experience in oncology nursing. • Strong interpersonal and communication skills. • Basic computer proficiency and ability to prioritize tasks. • Current CPR certification and oncology certification required within 2 years. • Ability to work independently in a fast-paced environment.
