Job Overview

We are seeking an experienced and compassionate Licensed Social Worker to join our team in a remote, full-time capacity. As a Licensed Social Worker at arenaflex, you will play a vital role in partnering with registered nurse case managers to provide telephonic case management and community resource navigation to our members. Key Responsibilities

• Collaborate with registered nurse case managers to assess, plan, and advocate for members' benefit plans and/or health needs
• Research and identify available options and resources for members with complex needs, including financial, transportation, safety, and housing challenges
• Provide telephonic support and guidance to members, empowering them to make informed decisions about their health and well-being
• Work closely with other clinical staff to develop and implement comprehensive care plans that address members' medical, financial, and behavioral health needs
• Utilize electronic exchange and other digital tools to communicate with members, colleagues, and other stakeholders
• Stay up-to-date on best practices and industry developments in social work and case management, applying this knowledge to continuously improve our services and support

Essential Qualifications

• Active and unrestricted LSW, LMSW, LCSW, or LISW license in the state of residence
• Master's degree in Social Work or a related field
• Minimum of 2 years of experience in medical social work or case management
• Strong knowledge of community resources, social determinants of health, and health care systems
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of a remote team, with a strong focus on collaboration and teamwork

Preferred Qualifications

• Experience working with diverse populations, including those with complex medical and social needs
• Knowledge of health care regulations, laws, and standards, including HIPAA and other relevant guidelines
