RN Nurse Navigator HFHP Per Diem- Member Outpatient Services
About the position Responsibilities • Coordinate the multidisciplinary team approach to connect patients with integrated services. • Collect and report data from patients regarding barriers, healthcare, medications, and prior treatments.
• Track, order completion, and follow up on patient care activities. • Monitor patient compliance with treatment plans. • Assist patients with referrals to outside resources and care coordination. • Participate in performance improvement activities and continuous quality improvement (CQI).
• Assist in the orientation, training, and evaluation of new staff. • Design or participate in the creation of health education materials. • Collaborate with medical providers and clinic management for patient and staff education.
• Oversee planning, scheduling, and implementation of clinical activities for navigated patients. • Serve as the central point of contact for navigated patients and their families. • Provide education and health coaching to patients and families on self-management of illnesses.
• Design and implement patient follow-up contacts and celebrate key treatment milestones. Requirements • AAS or above from an accredited School of Nursing. • Current Florida RN license free of restrictions or disciplinary action. • 3 years of nursing experience in specialty.
• Intermediate computer skills, including EMR navigation and Microsoft Office applications. • Ability to work autonomously with minimal supervision. • Strong attention to detail and critical thinking skills. • Strong communication skills and professional presence.
• Ability to maintain composure in a stressful environment. • Ability to work shifts at various times with on-call and weekend responsibilities.
