Develop and maintain positive relationships with individuals and families in need of wraparound services Conduct thorough assessments of clients' needs and create individualized care plans Collaborate with community resources and service providers to connect clients with necessary support and services Monitor and evaluate progress of clients' care plans, making adjustments as needed Provide crisis intervention and support to clients and their families as needed Advocate for clients and their families to ensure their needs are met and their voices are heard Maintain accurate and timely documentation of all client interactions and progress Attend and actively participate in team meetings and trainings to enhance skills and knowledge Keep up-to-date with current best practices and policies related to case management and wraparound services Work closely with other team members to ensure seamless coordination of services and efficient delivery of care Uphold the values and mission of the organization at all times and promote a positive image in the community.
