Clinical Case Manager Behavioral Health - Monmouth County

Conduct assessments: Conduct thorough assessments of individuals with behavioral health needs to determine their needs and develop appropriate care plans.
Develop personalized care plans: Collaborate with individuals, families, and healthcare providers to develop personalized care plans that address the unique needs and goals of each individual.
Implement care plans: Implement care plans by coordinating and monitoring services, advocating for necessary resources, and ensuring timely access to care.
Provide education and support: Educate individuals and families on their behavioral health conditions and provide support in managing their symptoms and improving their overall well-being.
Collaborate with healthcare providers: Work closely with healthcare providers to ensure coordinated and comprehensive care for individuals with behavioral health needs.
Monitor progress: Regularly monitor and evaluate the progress of individuals in the care plan, making necessary adjustments as needed.
Advocate for individuals: Serve as an advocate for individuals with behavioral health needs, ensuring they receive the care and resources they need to achieve optimal health outcomes.
Maintain accurate records: Maintain accurate and up-to-date documentation of all interactions, services provided, and progress made by individuals in the care plan.
Stay informed on industry developments: Keep up-to-date with the latest developments and best practices in the behavioral health field to enhance the quality of care provided.
Promote a positive and collaborative work environment: Foster a positive and collaborative work environment by actively participating in team meetings, providing support to colleagues, and promoting a culture of empathy and compassion.
