Job Summary and Responsibilities

In this vital role, you will be responsible for conducting routine scheduled visits with clients to monitor their care, oversee the work of CNAs and caregivers, and maintain detailed, relevant care plans. Your expertise will be invaluable in assisting our Clinical and Marketing Teams, and you will play a key role in coordinating client needs with their Primary Care Physicians (PCPs) as needed. Your responsibilities will include:

  • Conducting weekly and bi-weekly visits to clients, with in-person home visits as needed within a 2-hour time period
  • Performing new client assessments in a timely and efficient manner
  • Maintaining accurate and current care plans that reflect the unique needs and goals of each client
  • Providing counsel and guidance to family members on managing conditions and supporting their loved ones
  • Collaborating with our interdisciplinary team to ensure seamless care coordination and optimal outcomes

Requirements and Qualifications

To be considered for this exciting opportunity, you must possess:

  • An active RN License in Virginia or a compact state, with a strong foundation in nursing principles and practices
  • A valid Driver's License and reliable vehicle, with the ability to travel to client homes as needed
  • Current CPR certification, with a commitment to staying up-to-date on the latest developments in nursing and healthcare
  • At least one year of experience in home care, with a preferred background in case management or a related field
