As a key member of our team, you will be responsible for providing high-quality care and support to individuals and families in need. Your primary responsibilities will include: * Conducting comprehensive assessments of clients' needs and developing individualized care plans to address their unique challenges and goals. * Collaborating with clients, families, and healthcare providers to ensure seamless transitions of care and optimal health outcomes. * Providing education and support to clients and their families on topics such as medication management, health literacy, and self-care. * Utilizing advanced clinical judgment and decision-making skills to address complex clinical markers and risk factors that impact care planning and goal achievement. * Applying evidence-based practices and clinical guidelines to ensure high-quality care and optimal health outcomes. * Participating in interdisciplinary care planning and coordination to ensure comprehensive and coordinated care. **Key Responsibilities** * Conduct comprehensive assessments of clients' needs and develop individualized care plans to address their unique challenges and goals. * Collaborate with clients, families, and healthcare providers to ensure seamless transitions of care and optimal health outcomes. * Provide education and support to clients and their families on topics such as medication management, health literacy, and self-care. * Utilize advanced clinical judgment and decision-making skills to address complex clinical markers and risk factors that impact care planning and goal achievement. * Apply evidence-based practices and clinical guidelines to ensure high-quality care and optimal health outcomes. * Participate in interdisciplinary care planning and coordination to ensure comprehensive and coordinated care. * Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support. * Identify and address quality of care issues through established channels. * Collaborate with clients and their families to develop and implement care plans that address their unique needs and goals. * Provide crisis intervention and emergency response services as needed. * Participate in case conferences and rounds to ensure optimal care and outcomes. **Essential Qualifications** * Bachelor's degree in social work, psychology, or a related field. * Current, unencumbered licensure as an LPC, LMFT, LFP, or LCSW in the state of Oklahoma. * 3+ years of direct clinical practice experience post-graduate degree, e.g., hospital setting or elective care setting like wandering care or short-term center/office. * 2+ years of experience in PCs, keyboard navigation, and MS Office Suite applications (Teams, Outlook, Word, Excel, SharePoint, etc.). * Strong transportation and willingness to travel in-state up to half of the time.
