Registered Nurse-Remote Patient Monitoring (Outpatient)
About the position
Responsibilities
• Utilize health information and telecommunication to promote therapeutic relationships and shared decision making.
• Train patients on telehealth monitoring equipment.
• Conduct telephone or video enrollments.
• Monitor patients daily using the telehealth monitoring system.
• Perform frequent telephone patient triage and intervention.
• Provide counseling and education for health prevention and wellness.
• Counsel and educate on chronic, infectious disease, mental health, and weight management.
• Enter orders within approved departmental protocols.
• Collaborate closely with primary care providers and team members.
• Coordinate referrals of Veterans to community programs.
• Monitor and track program statistics.
• Initiate and assist in performance improvement activities.
• Participate in unit-based and organizational level Shared Governance councils.
Requirements
• 1 full and unrestricted license from any US State or territory.
• Experience in telehealth and remote patient monitoring.
Nice-to-haves
• Experience in care coordination and integrated care management.
• Knowledge of Veterans Integrated Services Network (VISN) benchmarks.
