We are currently seeking a Client Social Service Coordinator to join our team. The Client Social Service Coordinator is responsible for coordinating and disseminating information regarding a client’s total plan of care to all members of the treatment team while serving as an advocate for the client’s health, safety, and personal rights. In this role, you will monitor and assist clients in their day-to-day adjustment to the facility and routine daily living. You will also participate in the planning, development, organization, implementation, evaluation, and review of clients’ Individual Habilitation Plans. The position requires reviewing department policies and procedures and contributing recommendations for improvement. The coordinator acts as an advocate for clients’ personal rights, safety, health, and self-expression, while also serving as a liaison between clients, their families or guardians, facility staff, and community agencies providing services. Maintaining proper records in compliance with regulatory standards is essential, along with performing any additional duties as assigned by the immediate supervisor. Qualifications Qualified candidates should possess a bachelor’s degree (preferred) and one year of work experience in social services (desired). A valid non-probationary driver’s license is required.
