Job Summary

We are seeking an experienced Registered Nurse to join our team as a Remote Triage Specialist for our Primary Care Practices. As a key member of our Ambulatory Care team, you will be responsible for evaluating and assessing the healthcare needs of patients through telephonic evaluations, developing and implementing care plans, and coordinating services with other members of the patient's healthcare team. This is a part-time, remote position, requiring 20 hours of work per week, with a flexible schedule that includes day and evening shifts, as well as weekend coverage.

Key Responsibilities

  • Evaluate and assess the healthcare needs of patients through telephonic evaluations, using your clinical expertise to determine the best course of action for each patient.
  • Develop and implement individualized care plans, taking into account the patient's medical history, current condition, and treatment goals.
  • Coordinate services with other members of the patient's healthcare team, including physicians, specialists, and other healthcare professionals.
  • Provide education and support to patients and their families, empowering them to take an active role in their healthcare.
  • Document patient interactions, assessments, and care plans in a timely and accurate manner, using our electronic health record system.
  • Participate in ongoing quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practices.
  • Collaborate with other members of the Ambulatory Care team to ensure seamless care transitions and optimal patient outcomes.

Requirements

To be successful in this role, you will need:

Essential Qualifications

  • A minimum of 3 years of recent clinical experience in critical care, emergency department, home health, or physician practice nursing.
  • Telephone triage experience of at least 1 year is strongly preferred.
  • An Associate's Degree in Nursing from an accredited School of Nursing.
  • A current PA RN license or a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
  • Provider BLS (American Heart Assoc/American Red Cross) certification.

Preferred Qualifications

  • A Bachelor's Degree in Nursing (BSN) is strongly preferred.
  • Experience working in a primary care setting, with a focus on ambulatory care.
  • Knowledge of electronic health record systems and practice management software.
  • Experience with quality improvement initiatives and evidence-based practice development.
